If, after prayerful consideration, you determine that your child should attend Cornerstone Christian Academy, please come to the school office to formally enroll your child. You will be asked to complete an admission application and make an appointment for your elementary child to be tested. The registration fee of $75 and a current blue slip is die at this time and is non-refundable unless it is determined by the admissions committee that your child is not eligible for enrollment at CCA.

If your child is eligible for enrollment, the following forms and records will be requested and kept on file in the school office:

Admission Application (due with registration fee)
Signed statement of cooperation
Affidavit as required by the state health department
Student Health form
Photocopy of the student's legal birth certificate (from the Bureau of Vital Statistics).
Immunization Form (IMM-50, blue slip, or exemption form)
Copy of the student's social security card
Copy of current report card (if applicable)
Copy of student's latest standardized test scores
Proof of custody (if applicable)
Teacher Information Card
